Our Housing Stabilization Services in Minnesota
Home Assurance provides comprehensive Housing Stabilization Services (HSS) to eligible Minnesotans through Medicaid funding. Our services are designed to help individuals with disabilities, mental health challenges, or those at risk of homelessness find and maintain stable housing.
Do You Qualify for Housing Stabilization Services?
To qualify for no-cost Housing Stabilization Services through Medicaid, you must:
✔ Be at least 18+
✔ Be eligible for Medical Assistance (Medicaid) in Minnesota
✔ Have a disability or disabling condition (including mental illness, substance use disorder, or other conditions)
✔ Need support to obtain or maintain housing
✔ Be at least ONE of the following:
✔ At risk of homelessness
✔ Currently experiencing homelessness
✔ Transitioning from an institution or licensed facility
✔ At risk of institutionalization
✔ Currently residing in a transitional housing program
